Client Responsibilities:

Clients are responsible for providing accurate and complete information regarding their health history, current conditions, and any other relevant details necessary for the provision of services. It is essential to follow the guidance and recommendations provided by our healthcare professionals.

Payment and Fees:

Payment for services rendered by Thryve Florida Health and Wellness is required as per the agreed-upon terms. Any additional costs or fees will be communicated transparently before services are provided. We accept various payment methods and adhere to strict confidentiality in handling financial information.

Appointment Policy:

Clients must adhere to scheduled appointment times. Rescheduling or cancellation of appointments should be made within the specified notice period. Failure to attend appointments without notice may result in charges or impact future bookings.

Privacy and Confidentiality:

Thryve Florida Health and Wellness prioritizes the privacy and confidentiality of clients’ personal and health-related information. We follow stringent protocols and comply with HIPAA regulations to safeguard client data.

Intellectual Property:

Disclaimer of Liability:

While Thryve Florida Health and Wellness strives to provide accurate and beneficial services, we do not guarantee specific outcomes or results. Clients acknowledge that engaging in health and wellness activities involves inherent risks and agree to hold Thryve Florida Health and Wellness harmless from any liabilities.

Modification of Terms:

Thryve Florida Health and Wellness reserves the right to modify or update these Terms of Service at any time. Clients will be notified of significant changes through our website or via email.

Governing Law:

These Terms of Service are governed by the laws of the state of Florida, United States, and any disputes arising from or related to these terms shall be resolved in accordance with these laws.

Thryve Florida Health and Wellness – Informed Consent for Telehealth Services

Telehealth involves the use of electronic communications and technology to provide healthcare services remotely. As a client engaging in telehealth services with Thryve Florida Health and Wellness, it is essential to understand and consent to the following:

Nature of Telehealth Services:

Telehealth services encompass consultations, evaluations, treatments, and other healthcare-related services conducted remotely through secure video, audio, or other digital communication platforms. These services may not be identical to in-person care but aim to provide high-quality healthcare remotely. You understand that Thryve Florida health and wellness provides, through its Providers clinical care services through a web-based Platform as part of an overall primary care practice and that it is not a full-service medical practice. Our primary care services are herein referred to together as the “Services”. You understand that your use of such Services is voluntary and you can seek in-person treatment at any time. You further understand that the Platform may involve asynchronous and synchronous communications and the electronic transmission of medical information and other data between you and your Provider.

Benefits and Limitations:

Benefits of telehealth include increased accessibility to healthcare, convenience, reduced travel time, and the ability to consult healthcare providers from remote locations.
Limitations may include technological issues, lack of physical examination in some cases, and the inability to perform certain tests or procedures remotely.

Privacy and Security:

Thryve Florida Health and Wellness ensures the confidentiality and security of your personal and health information during telehealth services. We utilize secure and HIPAA-compliant platforms to safeguard your data.

SMS Terms & Conditions: 

1- SMS Consent Communication:

The information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

2- Types of SMS Communications:

If you have consented to receive text messages from Thryve Florida Health and Wellness, you may receive messages related to the following (provide specific examples):

Follow-up messages
Service Related Communications
Promotions or offers (if applicable)

Additional Options:

If you do not wish to receive SMS messages, you can choose not to check the SMS consent box on our forms.

Potential Risks:
There may be potential risks associated with telehealth, such as interruptions in communication due to technical issues or limited access to emergency care in case of technological failures.
Although rare, unauthorized access to transmitted information despite security measures cannot be entirely eliminated.

Client Responsibilities:

It is the client’s responsibility to ensure a private and secure location for telehealth consultations to maintain confidentiality.
Clients must provide accurate and complete information during telehealth appointments to facilitate proper evaluation and treatment.

Consent to Treatment:

By participating in telehealth services provided by Thryve Florida Health and Wellness, you consent to receive healthcare services remotely. You understand that alternative services may be recommended based on the assessment conducted during telehealth sessions.

You understand that by using the Platform to receive Services, you will not have an in-person physical examination from the Thryve Florida Health and Wellness Professional Entities that might identify a potentially serious medical condition, and that the absence of an in-person physical examination may affect the Provider’s ability to diagnose any potential condition, disease, or injury. You also understand and agree that the health information you provide through the Platform may be the only source of health information used by Providers during the course of your evaluation and treatment through the Platform, and that such Providers may not have access to any other health information held by your previous medical providers (e.g., allergies, drug reactions, etc.) or be able to otherwise confirm the health information you inform them about.

You understand and agree that any diagnosis you receive is limited, and, in some cases, provisional and that the Services are not intended, in all cases, to replace a full medical evaluation or an in-person visit with a health care Provider. Your Provider may not be able to diagnose or treat you if you require an in-person physical exam or any other test that requires a follow-up visit.

Any care that you receive is based on your symptoms and information you provide. You certify that the information, including your geographic location, you provide to the Platform for the Services is true, accurate, and complete. You understand that if you knowingly provide false, misleading, or incomplete information to a Provider, it may have a negative effect on your treatment and your health. 

Emergency Situations You understand that you should never use the Platform in a medical emergency. You understand that in a medical emergency you should dial 911 or visit an emergency room. Our services are not designed for acute treatment of severe behavioral health systems. In the event you are experiencing emotional distress, please contact the National Suicide Prevention Hotline: Crisis Text Line at 1-800-273-8255, text 988, or text “Home” to 741-741, to obtain immediate assistance.

Agreement and Acknowledgment: I acknowledge that I have read and understood the information provided in this Informed Consent for Telehealth Services. I agree to engage in telehealth services provided by Thryve Florida Health and Wellness and accept the potential benefits and limitations associated with remote healthcare.
